Do you accept insurance?
I do not participate in insurance plans. Depending on the details of your insurance plan's out-of-network benefits, I can provide an itemized billing statement that you may be able to submit to your insurance company for partial reimbursement.
When is payment due?
Payment is due on the day of your appointment.
Do you have a cancellation policy?
I have a 48-hour cancellation policy.

Is there parking available at your office?
There is street parking (meters, public parking garage) available nearby.

Do I have to climb the stairs to get to your office?
My office is on the 3rd floor. There is an elevator at the main entrance off
of S. Fourth Avenue.
